State of Nevada/Elko City Clerk (as Filing Officer) All City of Elko candidates must file a statement of financial disclosure with the State of Nevada Commission on Ethics AND the City of Elko City Clerk as required by Nevada Revised States (NRS) 281. The state form and instructions will be provided with the Candidate Filing Packet or you may obtain additional forms through the Nevada State Commission on Ethics Website using the link above. FILING WITH THE STATE: Candidates must complete and file the state form with the Nevada Commission on Ethics on or before Monday, April 16, 2007. If mailed, this form must be received in the Nevada Commission on Ethics Office by April 16. You must present or mail the completed state form to: Nevada Commission on Ethics We suggest you make two copies of you completed original form. Mail your original form to the Commission by certified mail, return receipt requested, to provide you with mailing documentation for your records. One of your two copies must be filed with the filing officer as noted below and the second of your two copies should be retained for your life. A candidate for public office or public officer who fails to file a Financial Disclosure Statement in a timely manner pursuant to NRS 281.561 is subject to a civil penalty and payment of court costs and attorney's fees. The amount of the civil penalty is: (a) If the statement is filed not more than 7 days late, $25 for each day the statement is late (b) If the statement is file more than 7 days late, but no more than 15 days late, $175 for the first seven days, plus $50 for each additional day the statement is late; (c) If the statement is filed more than 15 days late, $575 for the first 15 days, plus $100 for each additional day the statement is late. (NRS 281.581, Subsection 1). Knowingly filing a Financial Disclosure Statement containing false or misleading information is a gross misdemeanor. (NRS 197.130). FILING WITH THE ELKO CITY CLERK (AS FILING OFFICER): NRS 281.561 requires that a candidate for public office shall file with the Commission and with the officer with whom declarations of candidacy for the office are filed. Candidates wishing to file this copy in person with the Elko City Clerk must do so no later than 5 p.m. on Monday, April 16, 2007.
